I should have figured that you would blow a hole in my happy theory Kai.  :D

I've been using a spoon to scoop the Starsan foam off the top of the keg when I fill it.
Had a few people tell me it's not necessary. Now I have an actual reason
for doing so.

This is how I do it.

I basically do the same thing but I just use the CO2 to get the siphon started
and I don't have the fittings. I just hold the gas line up to the carboy cap till
the siphon starts. I stick a cotton ball soaked in alcohol in an air lock and put that
in the carboy cap to act as an air filter, I don't know if it actually filters anything but it
makes me feel better.

Why don’t you just figure out how much grain you need by assuming a standard 36 ppg for example? For this you need to know the efficiency, target volume and target gravity. If you want to be really precise with the extract potential of the grist you can calculate the weighted average by considering the extract potential of each malt and its grain bill portion.

Once you have the total grist weight the individual grain weights are easy to calculate from their percentages.

That’s how I do it.

  I know the percentages of grain I want,
but have to adjust them for efficiency.
  I tried using the method in Designing Great Beers to figure out
my grain bill. What this did was screw up my percents by weight.
I believe the book is giving me %'s by gravity per lb. ?
  So what I did was use the gravity per lb of the base malt
for all the other grains, so the weight %'s would stay constant.
  This seemed to work but I'm not sure it's the correct way to
go about it. Are there any easier way to scale grain bills for efficiency?

I can put all #'s up if any one cares???

I have a book with a recipe that calls for flaked rye.
I thought I might give it a try.
Any one ever use flaked rye?
I am guessing it would be much more of a subtle
flavor then using malted rye?

down my volumes, temps, times, etc.
got 89% efficiency which is way over

Sanitized a keg with starsan hooked up a cobra
tap to run the starsan out of the keg and to sanitize the tap and lne
I put a rubberband around the tap to hold it open
ran out the starsan filled and pressurized the keg
hooked the tap back up with out taking the rubber band
off.  Didn't lose too much beer but felt like a complete moron.

Do you remove the first wort hops or leave them in
for the whole boil ? ( I left them in)

I did it a couple times but I didn't care for the sediment it created in the keg.  In addition, in my case I didn't see any advantage to doing it.

Why would this produce more sediment?
