I do use hops when making a 5 gal. starter beer because I drink it (usually lightly hopped), but not when making smaller starters.

Next time think about brewing a 5 gal. 1.035 starter beer first to ramp up your yeast population.  As it turns out, these lower gravity beers are also very popular around my house.

Not sure how you are aerating your wort before pitching, but any method that results in a large foamy froth (i.e. pouring from bucket to bucket, shaking carboy, aquarium pump w/air stone) uses up some of the head forming proteins before fermentation begins.

I crush barley at .025 and wheat/rye at .015 and never a problem with my sparging.

I don't do a protein rest, but do lager my Kolsch for 6-8 weeks at 32Fto get the yeast (Wyeast) to drop out.
