No carbonation after 2 weeks in bottles

Hi all - brand new to home brewing!  Followed directions to the letter but after two plus weeks in bottles, we have no carbonation!  Flavor is great but it’s flat.  What did we do wrong?

What did you do?

Type of bottles?
Type of caps?
Amount of priming sugar/how was it added/how was it mixed?
Temperature of bottles for last 2 weeks?
